Step-by-Step Referral Process

  • Step 1: Initial Contact

    • Contact us via phone, email, or our online referral form to discuss the individual’s care needs.
    • Our team will provide guidance on available care services and funding options.

    Step 2: Needs Assessment

    • We conduct a free, no-obligation initial assessment to understand the individual’s care requirements, health conditions, and personal preferences.
    • This can take place at home, in a hospital, or via video consultation, depending on circumstances.
    • If referred by a healthcare professional or social worker, we work collaboratively to gather necessary information.

    Step 3: Care Plan Development

    • Based on the assessment, we create a tailored care plan that aligns with the individual’s health, well-being, and independence goals.
    • We ensure care plans are person-centred, flexible, and designed to evolve with changing needs.

    Step 4: Agreement & Funding Options

    • We provide a clear breakdown of care costs and discuss funding options, including:
      • Private funding (self-funded care).
      • Local authority funding (if eligible).
      • NHS Continuing Healthcare (CHC) or personal health budgets (PHBs).
      • Direct payments and personal budgets.
    • A service agreement is finalised, outlining the scope of care, schedules, and expectations.

    Step 5: Care Services Begin

    • Once the care plan is agreed upon, we assign a dedicated care team to support the individual.
    • Care services commence at a convenient start date, ensuring a smooth transition.
    • Regular care reviews and check-ins are conducted to assess progress and make adjustments as needed.
